U.S.-based healthcare professionals may use this form to submit unsolicited medical information requests to Vor Bio Medical Information. Responses will be provided by Vor Bio Medical Information, will be tailored to the specific request, and will be provided for scientific and medical information purposes only.

This form should not be used to report adverse events (AEs) or product quality complaints (PQCs). To report an AE related to a Vor Bio or RemeGen product or clinical trial, please contact Vor Bio Drug Safety within 24 hours at drugsafety@vorbio.com and vorbiosafety@navitaslifesciences.com. To report a PQC related to a Vor Bio or RemeGen product or clinical trial, please contact Vor Bio within 24 hours at VorQuality@vorbio.com. If an AE or PQC is submitted through this form, Vor Bio will route the report to the appropriate safety or quality function for review.
